1. Go to www.corvel.com
   
   
2. Click on “PROVIDER LOOK-UP” (top right)
   
   
3. Next to “Login,” type in “HARLEYSVILLE”
   
   
4. Click on the “LOGIN” button
   
   
5. Click on the “FIND PROVIDERS NEAR YOU” button (bottom left)
   
   
6. From the “Select a Network” drop-down menu, choose “Workers Compensation”
   
   
7. Enter “Zip Code” or your location (“Street address” is optional)

8. 5 miles is the default “Maximum Distance”; Choose another from the drop-down menu, if you wish
   
   
9. Choose “All,” the default setting, or select a specialty from the list provided
   
   
10. Click on “FIND PROVIDERS” and from the list shown contact the appropriate provider
